Become a Continuum Provider

Continuum has an extensive network of EAP providers throughout Nebraska, the Midwest region, and across the United States. All of our providers undergo a comprehensive screening process prior to joining our panel. Additional quality assurance procedures are used to ensure that all of our providers maintain the highest quality clinical services to all eligible employees and family members.

Continuum EAP is currently accepting applications based on the needs of our network.

If you are identified as a candidate for our EAP Network, you will be evaluated by our Provider Relations Department in strict accordance with the standards adhered to by Continuum EAP quality assurance measures.

Prospective Affiliate Provider
Does your practice have experience providing EAP services?
Does your practice have experience in substance abuse assessment?
Does your practice have training in Critical Incident Stress Debriefing?
Does your practice provide DOT SAP evaluations?

If we have a need for providers in your area, then we will contact you to discuss the credentialing process. If we do not currently need providers in your area, then we will add your information to our Provider Resource Directory and contact you when the need does arise.
